Our office does not file insurance​

OUR OFFICE CAN PROVIDE YOU WITH A RECEIPT OF PAYMENT MADE FOR SERVICES

OUR OFFICE DOES NOT PRE-CERTIFY SERVICES

WE DO NOT FILE INSURANCE OR RECEIVE PAYMENT FROM INSURANCE COMPANIES. 

Dr. Qualls is a licensed psychologist in the State of Tennessee who  has chosen NOT to participate on insurance panels outside of the hospital where he is on staff due to a variety of reasons including protection of client confidentially, and insurance company pre-certification requirements for services,  We believe insurance companies should not control your treatment; your clinical treatment should be between you and your doctor.   

Dr. Qualls' will be considered an "out-of-network" provider for services rendered. You should check your out-of-network benefits for mental health services.  Our office will provide you with financial receipts and proof of payment for services. 

Fees are due at the time of service and are payable by cash, check, or most major credit cards. 

WORKERS COMPENSATION CASE FEES  are handled in conjunction with the assigned workers compensation case manager and are NOT paid at the time of service by the client.

Psychotherapy session 1 Hour - $225

Neurocognitive testing Per Hour - $225 (typical evaluation is 8  to 12 clinical hours) 

Psychological and Personality Testing Per Hour- $225  (typical evaluation is 7-11  clinical hours)
